I affirm that all the information I provided is true & correct to the best of my knowledge & if any information I provided is incorrect or withheld it may result in my exclusion & possible legal recourse and/or negative publicity regardless of how long it takes us to discover the inaccuracies or intentional omissions of information.  I also understand that completing this application does not automatically ensure membership acceptance & that I must be approved first.  I understand that if I am accepted as a PANSOC Member or Volunteer that my image, pictures & videos may be used in promotional photos, videos, flyers, posters or other methods we use to promote our mission even after the event I were to ever leave this organization whether voluntarily or not.  I understand that this is currently a volunteer organization and thus I am not applying for a paid position.  I understand that Mil DD stands for Military Dishonorable Discharge or its equivalent in countries outside the United States.  I also understand that a yes to any of the fields relating to criminal history are not automatic disqualifiers as they will be reviewed on a case by case basis and that I must include the charges, cities, states, countries, dates & details of all convictions and current charges I was required to list on this application as well as any mental disorders or any situations in which I was found not guilty by reason of insanity and distinguish each charge I'm required to list between whether it was a conviction or a current charge.  I also understand that I will not list any charges that I was not convicted of other than current charges or charges that I was found not guilty of by reason of insanity.  I understand that the fields in this application had to be shortened because our website limits the number of fields and characters per field.  I understand that I must list N/A or Not Applicable or No in all fields that do not apply to me as all fields in this online application form are required to be completed, none can be left blank.  I also understand that an attached photo or photo ID are required with this application but a resume is optional.  I understand that my eSignature (Electronic Signature) & today's date are valid representations of my written signature & date & accept responsibility of its use on this application.
